3. Employee Identity Card (For Provisional Pensioner)
4. Mobile Number
5. Bank Pass Book
6. Only Gazetted officers of Central/State Government and Registered Medical Practitioners(MBBS) are authorized to certify Life Certificate.

Note: Will be received on and from 02/11/2024 to 30/11/2024 from 11.00 AM to 02.00 PM at the POURASHAKHA (near main gate, H.M.C.)
